Мало того, что сам сайт именуется www.ebi.ee , так еще и мыло есть, для связи с администрацией сайта - ebi@ebi.ee (еби, собака, еби её) Вроде, эстонцы всегда по-русски неплохо разговаривали, неужели настолько забыли все? Или это москали им сайт писали?

Caution advised for corticosteroid use in ICU

US research indicates that corticosteroid use in the intensive care unit (ICU) is associated with increased infection rates among patients.

Although corticosteroids have been used in the ICU for a variety of indications including sepsis, airway edema, and spinal cord injury since the 1980s, the popularity of such treatment has risen and fallen in previous decades due to conflicting reports on efficacy and safety.

"The current trend favors the use of corticosteroids, especially in the setting of sepsis and relative adrenal insufficiency," explains Rebecca C Britt (East Virginia Medical School, Norfolk, Virginia) and colleagues.

To investigate the matter in more depth, the team conducted a case control study, comparing the outcome of 100 ICU patients treated with corticosteroids between 2002 and 2003, with 100 control patients matched for age and injury severity.

Specifically, the researchers monitored the incidence of pneumonia, bloodstream infection, urinary tract infection, and other infections, as well as ICU length of stay (LOS), ventilator LOS, and patient mortality rates.

Multivariate analysis revealed that corticosteroid use was significantly associated with increased rates of pneumonia and blood stream infection, with odds ratios (ORs) of 2.64 and 3.25, respectively, compared with no corticosteroid use.

Britt et al also report that there were trends towards urinary tract infections, other infections, and mortality among patients who received corticosteroid therapy compared with those who did not, with ORs of 2.31, 2.57, and 1.89, respectively.

Logistic regression analysis demonstrated that patients in the corticosteroid group had a longer ICU LOS by 7 days, and a longer ventilator LOS by 5 days, compared with patients in the control group.

"Corticosteroid use is associated with an increased rate of infection, increased ICU and ventilator LOA, and a trend toward increased mortality," the researchers write in the Archives of Surgery.

"Caution must be taken to carefully consider the indication, risks, and benefits of corticosteroids when deciding on their use," they conclude.
